Total Triage
Important Notice for our Patients
ALL APPOINTMENT REQUESTS to see a GP will be triaged.
This will allow us to better understand your needs and enable us to signpost you to the right service. Routine Nurse appointments can be booked as normal.
- All requests received will be triaged and prioritised based on urgency.
- If your request is of a high priority, please make sure you contact the practice between 8am – 10am so it can be dealt with on the day.
- For routine requests please allow 2 working days for a response. You may be offered a routine appointment in 2 weeks’ time or signposted as appropriate.
You can contact the practice by telephone, email or via the online -econsultation form the reception team and the online -econsultation form will ask you a series of questions about your problem. Please make sure you provide as much information as you can and upload any appropriate pictures if required if using the form. This will help the GP triage your requests more efficiently, helping you get the right care as quickly as possible.
How to access the e-consultation form online –
- Via the NHS or MyGP app. You just need to go to services and then click on ‘contact your GP about a health problem’.
- Via our website – You need to click on the Online contact Form
This is a positive change and will help us move away from the 8am rush. It will allow us to manage demand and with your support, help resolve your requests quickly and easily.
As we are using a Total Triage model, it is very important that you keep your contact details updated . You can do this via the NHS app or by emailing us at surgerystaff.stonebridgehouse@nhs.net
All information given to our reception or clinical team is treated in confidence.
